Please note that submitting this form does not constitute authorization to return a unit to TTP.

Units that have been in service must have the Safety Data Sheets (SDS) for all service fluids submitted to TTP for approval prior to issuance of the RMA number.

Do not ship any unit to TTP without first receiving a Return Material Authorization (RMA) number issued by an authorized TTP representative.
